GROUP RATE FORM
This is your E-Registration for the 2006 School Of Puppetry Weekend Festival

Please follow these directions carefully:
If TODAY is the day of the festival you plan to attend, STOP!  It is too late to pre-register.  Just go to the location and register at the door.

  1. Print this page now.
  2. After it is printed, click HERE and start following these directions from your printout.
  3. You should now be at the "Group rate registration for a School  of Puppetry FESTIVAL" item page.
  4. Choose, from the drop down menu, the FESTIVAL you would like to attend. 
  5. Click "ORDER."
  6. Update the quantity to reflect how many people you wish to register.  (You must type in at least 8*.)
  7. If you want to participate in our eXtreme Puppetry 3xperience, click "keep shopping." If you do not wish to participate in XP3 skip to #9.
  8. Click the "Order" button below the "Sign Up for XP3" section.  You will be charged an additional $12, and automatically signed up for one person to participate in XP3.  (limit three)
  9. If you want your team to be evaluated by our staff, click “keep shopping." If you do not want your team to be evaluated skip to #11.
  10. Click the "Order" button below the "Pay your evaluation fee" section.  You will be charged an additional $15, and automatically signed up for evaluation.  
  11. If you wish to shop for other items, click "keep shopping." Otherwise continue the “check out” procedure as normal.
  12. After you complete the transaction:
    Write in your Yahoo! Order# here:_____________________
    Write in the total number of people registered here:_________
  13. When you receive your email confirmation: Print your email order confirmation and staple it to this page. It will show the complete address and phone number of the location you are attending, confirmation of your payment of registration fees, and evaluation fee payment confirmation. (if applicable) 
  14. Make a copy of this for your church or your own records.

 You are now pre-registered!  The FESTIVAL begins on Friday at 7:00pm, please arrive between 6:30pm and 7:00pm.  Saturday's schedule starts at 9:00am and lasts until about 2:00pm.

15. Bring these papers with you to the FESTIVAL for faster check-in at the door.
16.  If your team is participating in the evaluations, be sure to confirm the schedule upon arrival. 

 *You are purchasing this E-registration by using the group registration rate item on our web site.  This means that you should have registered at least 8 (eight) people to attend at the reduced rate of only $17 per person.  If after checking out, your Yahoo! order confirmation DOES NOT show a minimum of $136 in registration fees you will receive a phone call and/or email from our customer service department in order that we may reconcile the difference. 

Confirmation
You should receive an email confirmation of payment from Yahoo! Ordering.  You will also receive a confirmation email from Puppet Productions within a few days.  Please remember that the host church is not involved in the registration process.  Please direct all questions to 1-800-854-2151 between 8:00am and 4:30pm Central Time.  

Cancellations/Adjustments:
Cancellations must be made 10 days prior to the event in order to receive a refund.  Please call in to customer service at the above "800" number for any cancellation or adjustment.  Remember, the group rate is for registrations of 8 or more.  For example, if you register 10 people at the group rate, and then cancel 5 registrations, your refund will be adjusted to reflect the regular rate, for those actually attending.  Group rates can only be applied to groups of 8 or more. REGISTERING ONLINE gives you the benefit of less expensive registration rates, as a result, PLEASE take special care to register the correct amount of people.  We can not honor online registration rates over the phone.
